Waiver Agreement
Acknowledgment of Risk
I hereby acknowledge that I am voluntarily participating in DHHD hockey skills clinic and on ice activities being instructed by DHHD (Darrell Hay and Spencer McLean) at Idaho Central Arena in Boise, Idaho. I understand that ice hockey is a physically demanding sport that involves inherent risks, including but not limited to the risk of injury or even death.
Release & Hold Harmless
In consideration of being allowed to participate in these activities, I hereby agree to release, discharge, and hold harmless DHHD, Darrell Hay, Spencer McLean, and their respective agents, employees, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action, whether at law or in equity, arising out of or in connection with my participation in the DHHD hockey skills clinic and on ice activities. This includes, but is not limited to, personal injury, property damage, or wrongful death that may occur during my involvement.
Assumption of Risk & Rules
I understand and acknowledge that DHHD, Darrell Hay, and Spencer McLean make no representations or warranties regarding the safety of the skills clinic or on ice hockey activities, and I assume all risks associated with my participation. I also agree to abide by all rules and instructions provided by DHHD, Darrell Hay, and Spencer McLean during these activities.
Physical Fitness & Safety Equipment
I certify that I am physically fit and have no medical conditions that would prevent my safe participation in the skills clinic and the on ice hockey activities. I further acknowledge that I am responsible for wearing appropriate safety equipment and following all safety guidelines provided.
Binding Agreement
I agree that this waiver is binding upon me and my heirs, executors, administrators, and assigns, and that I am voluntarily signing it with full knowledge of its content and significance.
By signing below, you confirm you have read, understood, and agree to all terms above. If the participant is a minor, a parent or legal guardian must sign.
